What problem does it solve?

Documents completed bug fixes in a centralized, auditable format to record what changed, why, and how to prevent regressions.

Core Features & Use Cases

  • Append a fully formatted bug-fix entry to bug-fix-history.md with fields for Bug ID, Title, Root Cause, Files Changed, Fix Description, Services Affected, Regression Risk, Related Bugs, and Test Coverage.
  • Check for duplicates by scanning existing entries and update an existing entry if a similar event already exists.
  • Verify that all modified files are included in the entry to prevent partial fixes.
  • Support optional top regressions updates by inserting patterns into CLAUDE.md when applicable.
  • Provide a completion summary including a Bug Fix Logged block and a regression risk assessment.

Quick Start

Record the completed bug fix by invoking the bug-fix-log workflow to populate the required fields and append the entry to the history document.
