Prevents context loss during on-call shift changes by standardizing a handoff ritual that transfers tacit knowledge, current incident state, and near-term risks.

  • Synchronous-overlap handoff discipline: Runs a real conversation with a 15-30 minute overlap window to surface tacit knowledge and ensure questions land immediately.
  • Durable handoff document template: Captures the minimum operational ledger (open incidents, in-flight pages, deferred maintenance, noisy alerts, weather warnings, customer escalations, scheduled work).
  • Two critical handoff questions: Consolidates next-24h focus (“What should I worry about?”) and surfaces precursors (“What surprised you?”).
  • Weekend/holiday and mid-incident rules: Adjusts for altered coverage and recovery capacity, and enforces explicit tagging and acknowledgement so the off-going on-call does not leave until the next on-call confirms takeover.
