Guided evidence and observation capture for research workflows. Handles two modes: Mode A (Result note): structured evidence that advances a milestone — created when you have a definitive finding with metrics. Mode B (Observation note): lightweight mid-loop capture during investigate or iterate loops — created to record what was checked, found, or tried.

• Supports Mode A: result notes with questions, milestones, metrics, and confidence to document milestone evidence.
• Supports Mode B: observation notes for mid-loop findings and attempted actions without asserting milestone impact.
• Enforces validation and routing: prompts users to verify question and milestone IDs, creates notes via note_create, and updates plan/docs accordingly.
